Of patients suffering from depression, a minimum of 30% meet the criteria for difficult-to-treat depression. Repetitive transcranial magnetic stimulation (rTMS) targeting the left dorsolateral prefrontal cortex sites (dlPFC) maximally anticorrelated with the subgenual anterior cingulate cortex (sgACC) improves outcomes, yet lacks quantitative optimization frameworks. Using network control theory (NCT), we examined structural and functional controllability between dlPFC and sgACC in 972 participants (610 patients, 362 controls) from the MACS study. We computed controllability Gramians based on structural and functional connectivty for region-to-region control influence.
The dlPFC exerted control over sgACC (structural/functional), but more so over prefrontal/temporal-parietal and insular/cingulate regions. sgACC received maximal input from orbitofrontal/rostral ACC neighbors, not from the dlPFC. Control metrics based on structural and functional connectivity correlated (r=0.29,p<.001); control estimates were largely similar for MDD patients and controls. NCT partially supports anticorrelation targets and guides understanding of functional connectivity guided rTMS target selection.
